Prophylaxis against herpes simplex is usually recommended when doing procedures around the mouth because the trauma from the injections could potentially reactivate the herpes simplex virus (7). Therefore, it is imperative to ask patients prior to injection if they have a history of cold sores or fever blisters.

Techniques to reduce pain are critical to patient happiness and continued injections in the future. One of the best ways to retain patients and to have them refer their friends for treatment is to improve the patients’ experience. Ice, nerve blocks, vibration distraction, topical anesthetics, and mixing the filler directly with lidocaine can all reduce the patient’s discomfort (8,17,20). A good cosmetic result, few or no complications, and good pain management are the keys to success (Figures 10.23–10.25).
